Greek Leadership Village 

  • If you plan to have a vendor that provides a service (i.e. dunk tank, petting zoo, etc.)  GLV Vendor Request Form- COI Submission

  • The form will require that the chapter representative upload the COI and W-9 of the Vendor for review.
